CDM (Chronic Disease Management) is a Medicare program that helps people with long-term or serious health conditions access subsidised physiotherapy sessions. You can get up to five sessions per year with support from this program.
These sessions are based on a care plan made by your GP. This program used to be called the EPC (Enhanced Primary Care) plan.
How it works:
- Referral: You need a referral from your GP to access CDM physiotherapy. To be eligible, you must have a chronic condition that has lasted for six months or more. Discuss this with your GP.
- Care Plan: Your GP will create a care plan that outlines your treatment needs and refers you to health professionals, like physiotherapists.
- Medicare Rebate: Medicare covers part of the cost of each physio session. Nordic Physio offers gap free CDM consults on certain days/times. Bookings outside of these hrs incurs a gap fee.
- Session Limit: You can get up to 5 subsidised sessions per calendar year. These can be used for physiotherapy or other services like podiatry or occupational therapy.
- Re-referral: You’ll need a new GP referral each year to continue using CDM services.
